GDPR Consent

GDPR (General Data Protection Regulation) is the most significant piece of privacy and data protection in twenty years. It takes effect on 25th May 2018 and from that date we are required to ensure that we gain a new data protection and privacy consent from all clients. In it (amongst other things) we confirm what information we hold about you and how we are permitted to use it. GDPR is Europe’s new framework for data protection laws, replacing the 1995 data protection directive. This new framework came into force 25 May 2018.

All businesses holding personal/financial information must notify people and gain consent. At Bourne2care we are making you aware of the personal details we hold. Your name and email address will be put into our practice management system for Bourne2care Ltd only. Your financial information will be kept confidential.

Your GP will be notified of your appointment and will be informed of advice, recommendations, investigations advised and any medications prescribed.

No personal information will be shared with any third parties, except on grounds of safeguarding adults/children and if Bourne2care through discussions with those involved feel individuals need to be immediately referred to GP/Emergency Services if deemed at risk of harm/harming others and or for their own personal safety.

The majority of our consultations are via secure video conferencing. However; if you have questions relating to this, please do not sign this form and contact us . By signing you are consenting to video conferencing. Some personal information will be given to a registered online pharmacy, called Cloudrx, Their GPHC number 9011284. In order for them to dispense your medication, they will have access to some of your personal information. They will contact you and then your medications will be posted. If you would like us to arrange private blood tests on your behalf some personal information will be shared with Inuvi. We have a Business Manager  -  Rachel Brown, a Clinical Support Administrator - Helen Rising and Advanced Nurse Practitioners - Helen Elves and Lisa Davies in the team.   All of these people have signed Non Disclosure Agreements and will have access to some personal files.

In addition to this is Diane works closely with another menopause nurse practitioner called Hazel Hayden. She is CQC registered to provide private menopause/HRT services and in the event of illness will be the deputising registered manager. We collaborate to ensure patient safety, improving quality and leadership.  Consent will always be discussed, gaining full consent and cooperation.
